Gone fishin' is a simple Achievement and no title - though, it would be cool if it is one. ;)
I don't remember what you exactly have to do for the Achievement but here are my tips to you:
Rust Bucket Hat as alternative to the rain hat in your pic.
I don't remember which pole you need, I just checked CS, there are two different ones - use the one you didn't use before. :)
You must kiss doc's cheek first then u got achievement.
