This Involves Lots Of Trust
Buying item for 20 mil.....enter trade window, you put up your 9.9 mil and 4 or less items worth 10 mil total. Seller then puts up item for sale and everyone confirms. After that transaction is complete you then bring up new trade screen and buy back the items you put up in trade to begin with. If the seller logs out after first transaction it is still essentially a fair trade because you've only given a gold cap and 10 mil in items for what you wanted.
Edit: You can expand on this further with really big trades such as using esb or another expensive item as the collateral and then follow up with multiple trades after the "big" trade.
