Htiek (Very old AL player) has that title, it is not photoshopped.

An archaeological dig through the code reveals that this title was granted for the achievement "Purchased a premium character and earned the "Sentinel" title." which was removed in the 2012-11-05 Content Update (106846) update, just 3 days after the Canada and Australia limited release launch.

The achievement was removed, so you no longer get the 100 APs that was associated with it. However, we didn't remove the title.

BTW, if you scroll all the way down to the bottom (124 pages) of the in-game news (from the character selection window), you will see this update note. It doesn't call out the removal of the achievement, but this was the update when it was removed. All the AL update notes since the 2012-11-02 Canada and Australia limited release launch are still there in the in-game news.
