Nothing, just the one skill point that unlocks the piercer is enough in PVE.
Pvp you can add heal and 3 targets in case you want to heal more xD

You don't really have to add anything to the skill, but if you want to then the heal upgrade is definitely the only skill you should do.

Heal upgrade is very useful. You may find in the future it'll save you a couple times, so definetly do that one.
