I noticed in FH that when a mob was attacking and I was not, the mob was having a miss 'alert' appearing over their head. I assume this should appear over the target, and not the mob like damage or 'dodge' alert, or is this correct?
Miss over attacking mob, not character.

It's always done that. The way I look at it, It alerts you/mob that you "missed" your targ.
Yeah the one who attacks does the missing so it makes sense to me
