How is this done, do you drop from bosses or chests, and what are the chances?

You get the vials from the arena chests, not sure about the odds but its rare to find one

They also drop from mobs in planar tombs 3

I've done a ton ton ton of farming in arena and tombs. I've seen less than 5 vials ever drop in tombs, your best bet is to farm arena. Often (not always) I will loot I vial in ten arena runs. If you need to farm them I would focus on arena rt now.

Random! Set yourself a goal of ten arena runs a day and you will have those vials soon enough.

Like I said many times I run 10x and get lucky they some days it's nothing.
