Banish ring drops from elite Hugal. You can loot it with or without the Banish quest or complete antignome set.

As for Banish amulet, I believe you just need to be under the effects of Dimensional Bridge Potion when elite Shaman Pakuna is killed to get a chance for a portal where you can get the amulet.

You are incorrect about the banished amulet being in the elite gold heretic chests. The banished power sash belts are found in those chests. The only way the banished amulet can be looted is by using the dimensional elix when killing elite shaman pakuna.
