You only get a bonus when you equip at least two end-level vanity items. There are 3 of these vanities in game now, 1 could be acquired when the level cap was 56, and called 'crown of persistence', the second one was when the cap was 61, called 'shield of hallows' and the latest one is the lv 66 'mount fang armor'.

If you equip 2 of these vanities (doesn't matter which 2), you'll get a 'vanity set bonus', it will add following stats to your character: +5 dodge, +50 health and +10 armor.

Equipping the third vanity, together with the other two, would give you an extra: +1 crit, +5 health and +5 armor.

Edit: for all clarity, equipping 1 vanity item doesn't give you any stats increase.

Let me make that more clear, I'm not leveling to 66.

I quit leveling... I found out that L66 vanity is no stats without other vanities.

Keep in mind that you WILL get the bonus if you get the next end-level (71) vanity item also. Just letting you know, so you won't get disappointed when the next level cap would be released
