For me, I think it happens more when lots of skills are being fired off. Seems like what I see as alive isn't always avail for feeding. Could be they're now targets of other people's or my skills.
Happens a lot when we're killing very fast.
Could be graphics being behind the mechanics. For example, when a teammate is beating on the boss, I can stand right there, trying to feed and failing, until I see the boss die. It's possible, the teammates' hits were all processed before my feed attempts.
