Need help with Battle Report: Cycorp Labs quest

I have two of the three battle reports, but I can never seem to find the third one. Not sure if the boss drops it, or where it's at. I have the first one at the first level, and the second one at the second level. Third one = ???

You can grab the same one twice I think h

The way I have done it was find the obvious first one, and remake on the same map. Like Ak had said, it's technically picking up the same one, but it still counts towards the quest.
