None are scaled, I have heard of drops from the boss gauntlet and Skeleton Cult Crypts, they drop from the green trolls in the middle rooms.

As for drop rates, I am afraid I don't know, they are low though and I am not sure where they are on the drop table as far as rerolling for them, we could farm them to find out
