Lvl cap was 16 , but the crates one is complex , since they used to change contents of locked crates and release them , so there were different locked crates with different loot from different seasons , i'd say those early locked crates are the rarest.
If you're talking about chests , there are unopened chests with loot from early seasons still in the game , i have seen a pirate chest from kraken expansion (21) , which contains rare variants of demonlords , like purple , green and blue variants only available from those crates at lvl21
