When I am thrashing thugs sometimes my abilities don't work and I end up dead. Please fix this.

This bug has been known for a long time.
Apparently, the probability of it occurring increases when you try to time your skill to release at the moment it becomes fully charged.
Heals (for all classes) seem to be the most susceptible to this skill fail bug.
