"Shark Bouncer" arena achievement not awarded... glitch?

Does anyone else have this problem?
I have completed the "Shark Bouncer" achievement twice as described (Bloodhammer/Vortex under 3 minutes w/o dying), however I did not receive the achievement.
Is it glitched?
Or is there some other criteria that is not in the achievement description?

Did anyone in your party die? Sometimes that affects it, but it's random.
