Do you have to have the mirage robe, mirage staff, and OL cosmos helm for the bonus? Or can you interchange the Jacobs helm, keeper staff, etc and still have the bonus?

Yeah you need the mirage robe, staff and OL helm only. You can't interchange even one piece.
