Somehow I can't go to Nordr map even though killed Captain Bloodhammer... I have no clue why I can't go there. I don't want to kill the boss again. I hope I will get a response to this!

I don't believe defeating Bloodhammer affects if you're able to enter Nordr. To enter Nordr you must be at least level 23 and enter the gates and the bottom left of Kraag, after that you will have to defeat Jurn Woods and will gain access to Stronghold and the next dungeon.

Then you should be fine. Go to Kraag City and see the Herald of Arlor for the quest, then go through the big wooden doors at the end of town - not the hauntlet end, the other end.

May I know why you don't have the The Dead City achievement while you have The Kraken Isles achievement?

In preparedness for the possible next question.. You can access the Dead City dungeons by going to Kraag, heading towards the multiple Kraag Cannons and walk to the Dead City Gondola near the top right of the city. :D

My answer wouldn't work since I didn't see that Trance had already entered Dead City hehehe. So yup, either Trance didn't complete the quests or didn't complete all the dungeons.
