Upgrading an item

This happened to me twice already. I have a fang and got the 1st and 2nd slot fully grand gemed. When i the 3rd slot got grand, 1st slot went back to +3!!! I should've gotten all 3 slots fully grand by now. Happened to me twice already.

Oh wait, i used my mage in upgrading the item btw. Ign: Troyxxxx mage.

Seem like it doesn't revert to normal gem right? just moved slot? it happens to me before. i remember my grand gem was positioned at the top, and after crafting, albeit a failed one, the grand gem moved to center. overall, i still have one grand, but it does shift slot, and left me puzzled too. idk is this a real bug, happens to me quite a while ago.
