Juggernauts 50% chance for self heal question

Juggernaut only last for about 15 seconds so does the 50% self heal work after the 15 seconds has ended ( so for the whole cooldown of 45 sec) or does it only work in that 15 seconds of activeness??

jugger is good skill for tanking u get amazing skills ignore dmg etc for 10-15 sec but yeahh cooldown too long
