The duplicate disappeared after I closed the window and re-opened it. Was pretty funny.
This was result of I don't know how many attempts to friend each other and not having any friend requests show up.
I switched characters and logged in and out with no change in results. So I kept adding him.
I finally found him (how'd I do that? :P ) The first instance showed up when I added him and he was right next to me.
I still wasn't on his friend list and he still had no requests from me. He told me to add him again so I did.
For some reason when I added him again, a new instance showed up and his old one went dormant.

Lol I don't have a twin but a few followers out there. I mentioned in town that I wanted a clone and then 3 other kattatonic's with varius spellings popped up.

the friend list so is buggy... i hate using it.
WE NEED:
-account friend list, to not be confused by who is who (and scammers!)
-the # of people in their party
-FOLDERS

hahaa ohh weird! theres once when I invite my friend to my game, I keep ending up inviting myself. Then I had to pm her to join me instead.
