Can someone post how to get to the maps not on the world map?

I'll start
Cemetery road - portals in BC and FC, the signs are marked to the other town
forbidden crypt - cemetery road portal, near the turn. no sign
hidden passage 4... - rusty grate in BC, on way to cemetary rd.
hidden ice cave - icy grate behind vendor, head foreward as soon as you spawn in base camp.

I am not sure if I have made myself clear. Those are the ones I know. Please post ones that I have not posted or someone else has not posted.
