So I was at the boss in Honeycomb Hideout, the Vular Egg. I was using my usual tactic of standing inside the egg until the squidheads surround it, AOEing them all so they freeze, and then backing out. No problem. Normally the egg's health drops to a certain point—not 0—and it cracks open, finishing the level.
I'm not sure exactly what happened—maybe the final explosion of squidheads was enough to actually send the egg's health to 0?—but the egg disappeared completely, exposing a ladder. At this point, the enemy remaining count stayed at 1 (I checked that it was 1 when I entered the boss area), and the squidheads kept coming out!
(AFAICT, the ladder is not usable either, but there are several ladders in boss areas that are not actually usable.)
