I may be wrong but i think there are mobs on different levels in that area like down a ladder

ya, this is a known issue. i think they're still trying to pinpoint what is causing it. while battling, it's tough to notice when it starts happening. the problem is, you not only have to try to notice when it starts happening, but also what's going on when it does (where are you in the map, what enemies are being killed, what skills are being used, etc).

I don't know if it helps but I have always seen this happen upon joining an instance.
