I saw some guy wearing this and I wonder if it's some freaky awesome glitch of some kind.

Lol, yes it's real. Technically, it isn't one helmet. It's a glitch on the vanity + normal helmet and combines both, giving this look.
The new Lion-face Vanity + Glyph Helmet looks sick ;)
