You hunt the amulet from the boss in elite misty grotto.You have only a chance to loot it if you have a dimensional lix ticking.

You need the quest called "The Gap Between Dimensions". You can craft the elix after obtaining this quest from the npc in the secret room. In order to get that quest you must start the banished quest, and have the banished ring.
