I am not sure whether this is on the game side or if it is an issue with the iPhone OS. Also, I am running iPhone OS 4.0 Beta on an iPhone 3GS.
This does not happen all the time but when I am in game and get an incoming phone call, I decline the call. My game comes back and I go on my way. However, sometimes there is no sound in the game anymore but there is sound for the OS because my new mail sound still works.
Again, not sure which side this is on and it could be because I am running the 4.0 OS. Anyway, just wanted to mention it anyway if others were having the same issue.
Push notifications and text messages, so far, have not impacted the game or sound.
