I remember someone pointing this out before, happening in the Sewers, and the example I remember was Catacombs. Guess they haven't fixed it :)

When bears use Hell Scream the same thing happens. I first noticed it after purchasing my red night cap with the snow flakes popping out, really thought my vanity hat had a proc...:cower:
