I've noticed this as well, and believe it is due to multiple instances of the map available to balance load. If you reenter the town a few times, other toons should be visible. I've had times where a friend and I were in the same map next to the same NPC, but could not see eachother until joining through friend menu.

have also had the same issue. Its a double edged swd, load balancing v. annoyance of having to join sometimes rather than enter towne.

if you are trying to get to a populated town there is a search function under join games. You can even see how many people are in said town.
