I've been having the oddest issue, but as far as I can tell, no one else has posted on it... So, maybe it's just me.
The map is Cultist Crypt, Outskirts, the boss is cantor John Edward. Except, ever so often his name tag says Kobe Ikeda... He still acts as the cantor, fire whirlwind and all.
None of the executions have shown up in like 100 runs, so pretty much giving up on the map anyway :p
