I ran into this bug occasionally but once it cost me a legendary gem I got down to figuring it out. Here are the details of how to recreate this feat.
*This bug doesn't actually net you anything by being able to accomplish it, I'm explaining in detail so that STS knows exactly how it happens and can fix it. Also, I'd like players out there to know how it happens so they can avoid it if possible.
The setup:
#1: There needs to be a searchable item near the exiting circle.
#2: That item cannot be used before the boss is killed.
#3: A speed boost such as an elixir or adrenaline after feeding (optional).
When you get near the exiting circle there is a hand that appears in the bottom right of the screen where your attack icon usually sits and the same symbol appears when you're close to a searchable body. I can only assume that the code to the game cannot tell the difference between the two so when you complete one or the other the game automatically finished whatever process it thinks you were in the middle of. While running past the circle, you interact with the body on the other side and instead of completing the action of looting the dead corpse, you actually complete the exiting process and are forced to leave the map.
As it happened to me I was running circles around the boss like a chicken with it's head cut off (perhaps that was my mistake) and the very second I killed him, I passed through the circle, proceeded to pass over the body and clicked the action button one final time for good measure. It all happened so fast, but not fast enough for me to notice that a pink gem flew over head and landed inches away from my character. I sat at that mission complete screen for quite a while trying to move the few inches to get my legendary loot, but to no avail. That is what sparked my mission to ensure that this tragedy didn't occur to anyone else.
Ways to fix this bug:
Move all searchable bodies away from the exiting circle.
Change the way the code interprets how the exit timer works.
Add a confirmation button upon exiting a 3D mission.
As I said before, this is mostly just a heads up to the players out there. I find that this bug happens when you least expect it in the form of fighting a boss where the exiting circle will appear after he dies. While you're in the midst of a hack and slash fury, as soon as the boss dies, the circle appears and you happen to be close enough to a dead body full of gold and you click that action icon just one time too many and *poof* it's over just that quick.
