Is this title meant to be unlocked when having all 3 hardcore classes at level 86?
I've asked a few people who also have all 3 classes at 86 and don't have unlocked

You are correct! This will be fixed in a future server (content) update and we'll grant the missing title to anyone that should have received it.

The title is bugged. You can equip it but when you zone the title disappears.
