Charged combat medic sometimes fails

Recently, my charged Combat Medic skill on the rogue has been failing sometimes. After it's charged and released, the cooldown is triggered, but no health packs were created. To identify if this was due to lag, I waited for around a minute, and travelled the entire surrounding area to see if my health packs became "invisible". However, no healing were done at all. Thus, there is the case of charged health packs not appearing randomly.

I believe there is no "hit rate" to this skill, and it was working fine until about two to three days ago.
