It's not a vanity. You get from the crafter in FH.

Yes if you never did any quest in forest haven you can get it. First do a couple of quest offered in forest haven town then after wards the crafter guy will have his first quest, which gives the recipe to make that hat.
You can do it once per toon and also it's not a vanity it's a helmet with like 1 armor lol so it's just for looks in towns.

Trying doing the quest on a different toon that never did any quest. It should be the first quest you get from him
