Usually happens when I proc kraken armor on hedo. Kraken gun + belt + Kraken armor + enchanter amulet, switch zaar/smug+sb set 3/3 + pet (only crit damage pet). Event boss happens some too. Maybe...

I'm unable to repro this so far, I can drop a bunch of them with max haste.
