Juggernaut only last for about 15 seconds so does the 50% self heal work after the 15 seconds has ended ( so for the whole cooldown of 45 sec) or does it only work in that 15 seconds of activeness??

Only during the 15 seconds that it's active. ;)
jugger is good skill for tanking u get amazing skills ignore dmg etc for 10-15 sec but yeahh cooldown too long

as it mention above, it is only active when the skill is in effects :p
its a "must have" in pvp.
Good juggernaut is best tanks skill
I agree if u PvP jugg s must to tackle mazes n rouges sometimes
useless 50% heal chance pve pvp, just saying.
