Does the hammer drop only from the bosses? In which location of the dead city do you recommend farming?

best spot should be the mage's mine in arcanum grounds

Hammer of the Frost, at first. Sorry, couldn't keep myself.
Run in 3rd map of Dead City.
Hope you can get a chest!

Any mob in normal dead city maps can drop it, but it's very rare
