Upgraded armor bugged

Well i recently have done the mythic armor upgrade for my rouge toon.
After doing the chain of quests i turned in the final one to get my shiney new upgraded armor but that smelly old blacksmith slacked on his job and returned my armor without it being upgraded.
Now im on the mythic helm quest upgrade?
Im pretty sure this is a bug.

How can i fix this?
