Not a single drop rate in this game has ever been officially confirmed. The level 50 Vyxnaar helm is definitely much higher than 2.43% though; I believe it's an extremely high chance drop from the Vyxnaar that randomly spawns in Shadow Caves V (kind of like Pappy who drops Hooch Hats on Croc Feud Still Farms).

Level 25 Pink Conqueror's Vyxnaar and Vyxnaar Helms of Legend drop only from the Forgotten Crypt's Vyxnaar. It spawns randomly; it might not be in a particular instance of the map at all, or there could be multiple ones. You just need to keep running around the dungeon, and eventually one will spawn, or just rejoin the instance.

Accessing the Forbidden Crypt: the dungeon is not host-able; all games there have no host and thus no one can be booted. The instance shows up in the "all parties" Join Game list but it's not under the Host Game list. To get to the crypt, you'll need to go to Balefort Castle and turn backwards and walk into the right path, past the Grate. This should lead you to an instance that looks kind of like an old road. Go into the top portal and you'll find yourself at the Crypt.
