mythic helm didint upgrade T-T

i did the quest of 16 pieces but when i completed it i didnt have the helm it was in stash and now how i take the quest to do it again ?

after completing the pragment go back to kellys and talk to jerril

Same goes for my mythic amour. After I turned in the quest, I realized it was under gem upgrade so I cancelled gem upgrade but unfortunately, the amour was not upgraded and I cant get the quest again. Can someone please help?

I have a issue similar to this with my rogues mythic helm quest, i have the quest and have run thru windmoore harbor and skull cove about 10 times each now and havent got one single fragment even tried restarin the quest. It didnt help i would like to complete my rogues mythic upgrade as it took me very long to get.

What should i do? I tried the link above but my account info doesent worth there....
