Next level / Exit level issues

I dont know if these have been discussed in other threads but two similar issues I have been encountering (more than once) are

1) Sometimes when we finish killing all the mobs on a stage and i hit 'next level' it takes me to the next level but spawns me in some random spot and i find myself surrounded by 10 skeletons and am shortly dead. then i respawn halfway across the map at the normal exit.

2) Sometimes when we finish the last stage and I hit 'Exit level' instead of getting taken back to town, I find myself in some random level on some random map with some random people. Its really weird. When i exit level through the menu it works fine though.
