Those people are mostly using elixirs to add effect, event elix, hunter elix, spirit elix, love elix and such. For items Lunar Pendant, Archon Ring, Lepre, Luck arti, Blood Pendant. So far those are what i know that gives effect. Equipping Vanity set like Arcanestein and hiding it using options adds an effect too
