Execution Targets

I had thought that each of the targets for Execution achievements spawned randomly on a specific map. Mikael Woltorf's description says you killed him on the roof of a hospital, which I did. But now I just saw Mikael at the first 3d dungeon of Kain's Crest. So is it a bug? Or can they spawn anywhere?

I would love to know that as I have been farming svens mansion non stop and his brother is never there. I have, however found three pinks. Is there some trick to having these mini bosses show up? I do not understand why they are so rare.

Ran the same zone at least 50 times and only seen him once... I wonder though if a different target was meant to spawn there.
