Instakill in event

Why did the boss give me instakill at the event? I ran more than 50 times in hardcore mode. Suddenly the bos gives me instakill from 100% health to 0. It does not do that much damage, even in normal it does not kill anyone. In normal I have 40k points and I have not seen any instakill or extreme damage. Why did I kill myself in one hit without warning? Ign: Syniestro.

Happened to me just now but in normal, at one moment full hp next moment dead, without red zone or anything
