I don't know if the item is correct, but I found it quite suspicious.
I opened 80 chests in a row and didn't get any (pet level up). minutes later I opened 30 more and didn't catch any (pet level up)

Yes. Everything is okay with this item.
The Pet Level up is not very common in this chest.
