It depends. The heartseekers give you an HP bump while the lifethief gives you a Mana bump. Statistically, they are equivalent otherwise. Anecdotal evidence suggests that the lifethief is the preferred dagger for Rouges due to the Mana bump because it seems that we are always running out of mana.

People don't use Lifethief due to the mana bump. They use them because they've got way more damage output then heartseekers, even if they are satwise the same.

What I see is that heart-seekers have a more narrow damage range and lifethiefs have a much wider damage range.

That means that your high-end damage will be superior, but your low-end damage could be much worse. Personally, I use heartseekers for standard maps and watchmans bows or lifethiefs for elite maps.

The choice becomes consistent damage with heartseekers or potentially higher damage and crits with lifethiefs.
